Thank you for your interest in a PERC community building or restorative circle.
The goal of the Promoting Equity and Relationships amongst Colleagues (PERC) program is to promote healthy relationships of staff and faculty by providing space where relationships can be built or repaired. We invite you to share some information about the relationship(s) you are hoping to build or repair below. Please note that this information will be shared only with PERC Fellows, who like you, are mandatory reporters for Title IX/DHR. If you have any questions and would like to discuss the PERC process in advance of submitting a concern or issue here, please feel free to reach out to us at PERC@SFSU.edu.
Once you have started your responses, you will be able to go back and revise sections, but you will not be able to save your answers to continue at a later time. So, do make sure that you will have the time to complete the questions once you have begun. We anticipate that it will take roughly 5 min to complete if requesting a community building circle and 20 min if requesting a restorative circle.
